Thank you for your interest in partnering with Fair Chance!

How to fill out the application: In completing this application, please be candid and provide up-to-date information so that we may fully understand the challenges and opportunities facing your organization. This will enable us to determine how a Partnership would best benefit you. (Please note that this information will be treated as strictly confidential and shared only with our small, internal selection team.)

There is one additional form that must be completed and submitted by your Board Chair. Please forward the following link to your Board Chair so that they can complete and submit the form.

Please provide information about any national or international affiliations.
An affiliation is a formal relationship codified by a memorandum of agreement by which your organization is closely associated with a larger organization. This could mean that you are a part of the larger organization, or that you are an independent 501(c)(3) that is closely associated with a national or international organization. This type of relationship has many names, including chapter, network, member, subsidiary, etc.

For example, Groundwork DC is an independent 501(c)(3) that is part of a National Groundwork Organization - this is the sort of affiliation that we want you to include. Public Allies DC is a chapter of a National Public Allies Organization - this is the sort of affiliation that we want you to include. Fair Chance is a dues-paying member of the Alliance for Nonprofit Management - this is not the sort of affiliation that we want you to include.
