Career Changed dogs are typically 12 to 18 months old and have undergone professional training; however, they are released from our service dog program due to various factors, such as behaviors incompatible with service work or health considerations. When these dogs transition out of the program, disclosures will be provided regarding the reasons for their career change. 

Our dogs receive comprehensive medical care during their time with us and are up-to-date on vaccines and flea and heartworm treatments. If a dog is too young for spaying or neutering, arrangements will be made. 

We match these dogs with applicants based on compatibility, not when applications are received. Applicants will get a two-week trial period to see if the dog will be a good fit.

The adoption fee is $2,000 and is subject to change, based on the circumstances surrounding dismissal as well as the level of training attained. Full payment is required at the time of adoption; unfortunately, we are unable to offer payment plans. 

A non-refundable $55 application fee is required with the submission of this form.

Are you willing to adopt a dog with the following conditions? Please note that a lot of these conditions will not severely hinder a dog's quality of life, but due to our strict level of health requirements, they cannot be placed as service dogs.





Your Commitment

JLAD requires adopters to provide the necessities which include food, water, bowls, shelter, bed, toys, collar, leash, treats, veterinary care, etc.

JLAD prefers our dogs are left alone no longer than 5 consecutive hours.

Caring for a career change dog requires a commitment for the life of the dog.

If accepted, I am able to pay the $2,000 adoption fee and am financially able to provide for my career change dog and his/her ongoing needs.
