I give permission for the photos/videos to be used by Fight Bladder Cancer in their materials. I can also provide permission from anyone else shown in the photo(s)/video(s).
I understand that:
• Fight Bladder Cancer may use the photos/videos supplied on their website and on social media, including YouTube
• Fight Bladder Cancer may also use the photos/videos offline in their printed materials, including, but not limited to, Fight magazine, Patient Information Booklets
• Fight Bladder Cancer reserves the right to use the photos /videos without prior notification
• The photos/videos may be used for promotional materials, publications, articles and potentially for publicity/marketing purposes, including, but not limited to, Bladder Cancer Awareness Month
• Photos/videos may be shared with media or local press for cases, including, but not limited to, Bladder Cancer Awareness Month
• Once published on the Fight Bladder Cancer social media and website, my image may be shared by other accounts on social media, and that Fight Bladder Cancer has no control over this
• I can withdraw consent for this at any time. We can email
info@fightbladdercancer.co.uk if we change our minds (note:
this will not apply to published material. Fight Bladder Cancer cannot control already published material or recall it)
We agree that:
• We will not be paid a fee in respect of any use made of the photograph(s) and/or video recording(s) but have the opportunity of being involved in promoting Fight Bladder Cancer in the manner set out above
• Copyright in the image(s) and/or video recordings will be transferred to Fight Bladder Cancer
• Fight Bladder Cancer shall have the right to edit, modify, crop, add to or subtract from the photographs and/or video recordings at its entire discretion and without my approval
I have read and understand the conditions and consent to my images and or video recordings being used as described. I understand that the details we provide and any photographs or film will be processed under the Data Protection Act 1998, the General Data Protection Regulation and any future legislation.
