iFoster partners with T-Mobile to provide smartphones to youth impacted by the child welfare system. Choose to pre-pay 6, 9 or 12 month increments of service. Service includes unlimited talk, text, 4G LTE data and 10 GB of 4G LTE hotspot data.

Phone: New Android smartphone - $150 includes all taxes and shipping
Service plan:  No carrier contract. Choose to pre-pay 6, 9 or 12 months and you can re-up as needed before service period expires. Cost is $30/month includes taxes and fees

After completing the iFoster National Phone agreement, please complete all sections of this application. Please share the application with the youth and provide them with the Terms and Conditions of receiving technology from iFoster. 

YOUTH APPLICANT INFORMATION (iFoster will have the youth's info, but they will remain anonymous to T-Mobile) 






Shipping Address: Please provide a safe location for us to ship the phone. We can ship it to an agency point person, directly to the youth or a supportive adult. For privacy purposes, we can not use a youth's name if shipping to a youth, so we will need a nickname or the name of a caregiver/supportive adult for the shipping label. 








Billing & Shipping Information
After reviewing submitted application(s), iFoster will email an invoice to the Agency point person designated on the iFoster National Phone Program agreement.  Once the invoice is paid, T-Mobile will prepare and ship the phone to the shipping address provide above. iFoster will email shipping tracking to the shipping contact info above.

TERMS AND CONDITIONS

The Applicant pledges the following:

·       Applicant will not sell or give away their smartphone (device).

·       If device is lost or stolen, Applicant will file a police report and contact iFoster immediately at 1-855-936-7837 or support@ifoster.org.

·       If device is lost, stolen or broken, the applicant may obtain a replacement new or refurbished device at cost through iFoster ($150).

·       Applicant will not use their device or telecom service for illegal activities including, but not limited to cyber-bullying (the use of electronic communication to bully a person, typically by sending messages of an intimidating or threatening nature), sending/receiving nude pics of minors, solicitation (the act of accosting someone and offering one's or someone else's sexual favors in exchange for money, food, housing, drugs or clothing), hate messages or posts (abusive or threatening speech or writing that expresses prejudice against a particular group, especially on the basis of race, religion or sexual orientation), etc.

·       Applicant will call or email tech support (1-855-936-7837 or support@ifoster.org) if they encounter any hardware or software issue with their phone or telecom service issue.

iFoster pledges the following terms:

·       iFoster will provide Beneficiary with a functioning device for their sole use.

·       iFoster will provide Digital Life course as an app on the phone which will help the Applicant take care of the phone, understand safe use of the provided telecom service voice, text and data capabilities and know who to contact in the case of any issues.

·       iFoster will provide ongoing, free tech support via toll free (1-855-936-7837) or via email (support@ifoster.org).

·       iFoster reserves the right to take back the device and accompanying telecom service should the Applicant fail to fulfill their commitments as outlined above.

If the Applicant does not fulfill the above obligations and/or for reasons determined by their social worker or attorney or their designee, the Applicant should no longer have the device and service, the Applicant will be required to return their device to iFoster or iFoster’s local designee and their telecom service will be terminated. Additionally, Applicant may be asked to participate in a survey asking them to provide feedback on the program and the benefits (if any) including academic, social well-being and use of the Internet for connecting with others. Participation in the survey is voluntary.

Thank you! We will review the applications and email you an invoice. Once the invoice is paid, T-Mobile will prepare and ship devices to the agency address provided above. iFoster will email you with shipping tracking information.
