Thank you for your interest in attending the National Jack Summit! If you are filling out this form, it means you are applying as a delegate to the summit.

We’ll be delivering our 10th installment of National Jack Summit in a hybrid format, with programming and activities being offered both virtually and in-person. Of course, we will be keeping an eye on public health recommendations and safety guidelines and will make changes accordingly, if necessary. We’re beyond excited by the possibility of coming back together, and we hope that you are too!

Here are some details we are pleased to share:

  • A portion of delegates will have the opportunity to travel to Toronto and experience the summit in-person. We’ll want to ensure we have diverse geographic representation with those who attend, as this is such an impactful part of our National Jack Summits
  • We’ll be using Hopin as our event management platform, helping us create a seamless and connected experience for delegates attending virtually and in-person
We are committed to making the National Jack Summit experience a meaningful one for Francophone youth. In addition to making live translations available through our event platform, you can expect bilingual speakers, dedicated space to connect with fellow Francophones about goals and ideas, and other pieces of programming offered in French.
When filling out the application below, we encourage you to think outside the box and really reflect on what you have noticed and observed not only in your lived experiences, but what you have noticed and observed around you. If you are looking for help in jumpstarting some of your thinking, we highly recommend checking out the recently published Jack.org Youth Voice Report, a summary of the experiences and perspectives of 1,107 young leaders from across the country.

Demographic Info
In this section, we will ask you questions about how you identify. In our commitment to equity and inclusion, we are gathering this data to help us (1) better understand the youth we work with, (2) identify the gaps in our programming, and (3) build pathways to break down barriers to participation. This data will also allow us to represent what we learn through our surveys and represent those findings more accurately. We understand that can be sensitive information, so we want to reassure you that this information will remain confidential and secure. Please feel free to select ‘prefer not to say’ if you feel uncomfortable answering any of these questions.
