Mentor Registration Form

Thank you for signing up to be a Creative Access mentor. We run multiple mentoring programmes annually for candidates from groups that are under-represented in the creative industries in terms of ethnicity, socio-economic background or disability.

Mentors / mentees typically meet once a month for six months. We'll provide you with mentor training, guidelines, a mentoring agreement and will support throughout the process.

Please fill in this form with as much information as you can, so we can make the best possible match.

By completing this form it does not guarantee that you will be matched with a mentee
If you are matched, it may take some time for us to pair you with someone. Priority is given to our employer partners.

We will only share you name, role, email and friendly introduction with the mentee.

Some mentees request to be mentored by someone with similar lived experience. It would help us if you could share any relevant information that we can use during the matching process.

This is considered sensitive personal data (also known as special category data). By giving us this information, you understand and agree that you are giving us your explicit consent to collect, store and use this data. This will not be shared with your mentee. There is an option on all questions to choose 'Prefer not to say'. If you want to change any responses, you can contact us to do so.
