Student Participation Waiver, Disclosure Form and Media Release

I understand that all active sports may result in injuries. I release from responsibility the employees, officials, coaches, directors, members, volunteers and representatives of The Baseball Inc. (“The BASE”) of any liability, claims, or demands which may arise out of my child’s participation in practices, games, or any other functions or programs of The BASE, at The BASE Club House and/or any other fields and/or venues used by The BASE to deliver its programs and services.

In the event of a medical emergency, if a parent or guardian is not present, I give The BASE permission to administer first aid and/or call an ambulance for treatment and transport to a local hospital.

I also give permission for The BASE, partners of The BASE, and the news media to photograph, videotape, and/or interview me and/or my child in conjunction with The BASE. I understand and acknowledge that any such photograph, videotape, and/or interview or portions thereof will be used for public view or distribution, and I grant The BASE and the news media any and all rights to use thereof without consideration.

I acknowledge that I have read and understand the above statements and that I am of legal age to execute this release and waiver.
