Thank you for taking this time to give me feedback about your experience, or that of your child in the case where you as a parent or perhaps as a team Leader of a training &/or coaching/mentoring group -has engaged me, Kerry @ Sydney Hypno for therapeutic &/or coaching services.
This feedback is very valuable to me as I always seek to better understand how I am doing - based on your comments and feedback.
This form allows me to hear back from you in real terms about you, your experiences and about your treatment plan, how you have engaging both short term and longer term, with your tasking and working towards your therapeutic outcomes.
I would like you to give me some insight as to how our working together has assisted you and how I could continue to improve and do better by getting your feedback.. So please do give me this information so I can improve my work and work towards even better client outcomes.
As always, It is always my aim to really assist you to help you to get the results you want.
I seek to better understand you, and like you, wish ensure you received the best outcomes from the time you invested and want your best outcomes to continue.
In order to serve you better, and to perhaps assist me to help serve both you and others better in the future, I ask for your considered observations & input. I am asking you to determine what elements you would like more of, and even those elements you might want , less focus on, for your therapeutic journey to be the best it can be.
As the unique individual you are, knowing how to best reach you, to best provide you with relevant insights into your processes will always helps me to design a tailored program to identify the goals and outcomes we work together in our therapeutic alliance to achieve.
You may be completed in that therapeutic process, or perhaps still on your changing and improving path.
I am happy for any feedback you have to share with me right now - and also - of course - to engage again if you wish to do so at any time in the future.
Initially, you will have completed a client intake form, and all those elements you read about confidentiality and duty of care still apply of course to feedback I receive.
