Complete applications are due to the Foundation no later than 5:00 PM on April 1. This form must be completed and supporting documents must be uploaded or postmarked (if mailed via US Mail) by the deadline. Supporting Documentation
The following supporting documents must be submitted to the Foundation in order for your application to be considered. Incomplete applications will not be considered.
SECURITY NOTE: 
Please mark out ALL social security numbers listed on submitted forms 
to help ensure the security of your and your parents’ identities!

You are responsible for having a copy of your official high school transcript submitted to the Foundation by the appropriate school personnel. The transcript should cover the first three and one half years of high school through the end of the Fall term of your Senior year. (Unofficial transcripts will not be accepted.) It should include a list of courses you have taken and the grade received in each course, your class rank and SAT® scores.

You are responsible for submitting a copy of your of your Federal Student Aid Report (SAR). You must complete the Free Application for Federal Student Aid (FAFSA) form (www.fafsa.ed.gov) to receive your Federal Student Aid Report (SAR). This form can be completed as soon as January 1st of the year you plan to attend college. You can go back to your FAFSA to check status, make corrections to a processed FAFSA and get other information should your circumstances change. Four to six weeks after submitting your FAFSA, you will receive your Student Aid Report (SAR) which summarizes the information you reported. (A sample SAR is available for your reference on our web site at www.crawfordheritage.org/docs/CompletedSARsample.pdf.) Applications missing your SAR or other supporting documents may not be given the same consideration, so please plan ahead! Contact your guidance counselor for more FAFSA information. If your SAR is not available, list the reason in the space below.

Enter in the box below the Expected Family Contribution (EFC) number from your Student Aid Report (SAR). If your EFC is not available, list the reason.

You are responsible for submitting a copy of your most recent IRS Form 1040 and one for each parent/guardian. If either parent/guardian fails to supply an IRS Form 1040 you must include a statement indicating your effort to obtain the form and the contribution toward your education expected from that parent. PLEASE STRIKE OUT sensitive information such as Social Security Numbers and Bank Account information.

Letters of Recommendation may be from a guidance counselors, teachers, school administrators, church representatives, supervisors or others who may adequately describe your academic achievement, character and/or work ethic. They may not be from relatives. Individual scholarships may require Letters of Recommendation from specific people, these can count toward the total of three required letters.
Standardized Test Scores - SAT® (OPTIONAL)



Essay

The essay should describe your career objectives and the contributions which you expect to make to the public good while achieving those objectives. It may also describe your academic accomplishments and participation in extracurricular activities, However, it should not be redundant to the lists of scholastic honors, extracurricular activities and memberships submitted in previous sections of this application.
