Participant hereby consents to use of their likeness, voice, sounds, written interviews, and biographical information by Carolina Farm Stewardship Association (CFSA) and its successors, affiliates, licensees, and assigns (collectively, “Carolina Farm Stewardship Association”).
Participant hereby grants Carolina Farm Stewardship Association—and its successors, affiliates, licensees, and assigns (collectively, “CFSA”)—a license to use, edit, reuse, and authorize other parties to use transcriptions, photographs, film, video or other recordings (including their name, likeness, voice, sounds, written interviews, and biographical information) as CFSA may desire. This includes, but is not limited to, non-profit purposes such as use on its website, program evaluation, inclusion in fundraising materials, media articles, and any other media, whether now known or hereafter devised, in perpetuity, without acknowledgment or compensation to me.
To the fullest extent permitted by applicable law, Participant releases CFSA from responsibility, liability, claims, demands, and/or damages (actual and consequential) of every kind and nature, known and unknown (including, but not limited to, claims of negligence), arising out of or related to disputes between users and the acts or omissions of third parties.
By completing this release form, I hereby represent that I have read the foregoing Release and Authorization, and fully understand the meaning and effect thereof, and intending to be legally bound, I have signed this Form digitally on the date set forth below.
