Media Consent

I give permission for photographs and video of my child to be used for the purposes of promoting the work of the Kenelm Youth Trust on social media, our website and other promotional materials.

Medical Treatment Consent

In the event that I cannot be contacted by ordinary means, I give permission and consent for my child/young person to receive any necessary medical treatment and authorise the group leader/person in charge to sign any medical documents required by the hospital authorities.

Day Trip Consent

I give permission for my child to participate in the day trips organised which will be off site.

I consent for the young person to attend the day trip to Alton Towers.I consent for the young person registered to attend the day trip to a water sports activity centre.

If you are travelling from a diocese in Scotland or from the Northampton Diocese we would like to share the information you have provided in this form with your diocesan or parish youth leaders who will be accompanying your child for Summer Camp.

COVID Lateral Flow Testing

The safety of your child during Summer Camp is paramount to us. To be able to attend you will need a NEGATIVE Lateral Flow test result on the day to attend Summer Camp.

All Lateral Flow Testing results are for our information only and will not be shared outside of KYT.

My child will perform a COVID test before arrival at camp and share the results with KYT.

I understand if my child produces a POSITIVE test, they will be unable to attend camp.

Travel Insurance

Due to the possibility of your child being unable to travel and participate in Summer Camp at the last minute or if they test POSITIVE, we strongly recommend you consider taking out a travel insurance policy (prices start from approx. £5.00) as we will be unable to offer refunds for these reasons.

Payment

To complete your registration a non-refundable deposit of £50 is payable now. The balance will be due by Friday the 1st of July. Once you have clicked submit there will be a payment link provided on the confirmation page where you will be able to pay by card.

Gift Aid

Boost your donation by 25p of Gift Aid for every £1 you donate

Gift Aid is reclaimed by the charity from the tax you pay for the current tax year. Your address is needed to identify you as a current UK taxpayer.
