The Reaching Home grant is a one-time, short-term investment. A project is considered sustainable when either the project activities or benefits achieved as a result of the project activities continue after the grant has ended. What other sources of funding are in place for future service provision? *Note: The "other sources of funding" tab of your budget must include details reflecting the amount of funds and project activities that will be supported by any other funders.

Will you plan to evaluate your project and determine its impact and outcomes?
