BY COMPLETING AND SUBMITTING THIS FORM, I AGREE TO THE FOLLOWING:
*I authorize my daughter’s participation in
recreational and/or educational activities with the Chicas Program.
*I understand that personal information provided
to the program about my daughter and family may not be released to anyone
without my written consent, unless it involves my child’s safety.
*I authorize my daughter’s participation in
surveys and evaluations, which may be used to determine the program’s
effectiveness and/or to promote the program.
*I understand that Chicas’ personnel are
responsible for overseeing my daughter during an event/activity, but they are
not obligated to oversee them after the event/activity ends. I agree to be responsible for picking up my daughter or finding someone responsible to do it. Also, I will communicate
transportation means to my daughter and Chicas’ staff.
*I understand that the program takes the proper precautions to secure
participants safety at all times. I agree not to hold the Chicas Program or its
volunteers liable for any accidents or injuries to my daughter while she is
participating in the program.
