You will have recently attended an assembly run by one of the Mind and Body practitioners from We Are With You, in this they will have explained why we are asking young people to complete the survey and what will happen after the survey has been completed. Please answer questions honestly as they are designed to highlight whether you may benefit from support.

 

You should have been told that this survey is confidential but not anonymous, as your school will be able to identify young people’s names from the ID number you will have been given. 

 

We do this so young people who need or want further support are able to access it and those who could benefit most from the service are offered the opportunity to attend the sessions. No information will be shared beyond the Mind and Body service and your school without talking to you first. We will only share information if we have concerns for yours or someone else's safety.

 

Most importantly you do not have to attend the programme if you do not want to.

 

Anonymised data from these surveys will be published to help us highlight what young people think and feel about the topics covered in the survey. N.B. This data will never identify an individual’s response to a question.
