Shaw Trust intends to submit a tender for the Community Mental Health and Wellbeing Service - Live Well Kent & Medway and would like to hear from organisations delivering mental health, wellbeing and housing / accommodation services in Kent and Medway.

The Opportunity

KCC, K&M CCG and Medway Council are working in partnership to commission an integrated Community Mental Health and Wellbeing service. This service will ensure compliance with statutory responsibilities and offer a consistent and equitable service across Kent and Medway.

The model sits outside of, but will work closely with secondary mental health services and will form a key part of an integrated pathway across the voluntary sector, primary care mental health and social care and include public health initiatives to ensure there is appropriate, equitable, timely and cost-effective interventions for vulnerable people in the community

Key aims of the service are to; aid recovery and prevent relapse, improve health and social care outcomes for individuals with poor mental health and wellbeing, prevent suicide and reduce the stigma of mental illness.

 

Health and Care Partnership Area Lots

Lot 1: Dartford, Gravesham, and Swanley Health and Care Partnership Area

Lot 2: Medway and Swale Health and Care Partnership Area

Lot 3: West Kent Health and Care Partnership Area

Lot 4: East Kent Health and Care Partnership Area

Lot 5: Management of 24hr Telephone and Online Support - Kent and Medway Wide Service Specific Lot

 

The following Services form the core of the new programme:

  • A delivery Network of Activities to improve Clients Well-being
  • Support to Enable People to Secure or Retain Accommodation – Kent Only
  • Mental Health Housing Related Support – Kent Only
  • 24/7 Telephone and Online Support – One Strategic Partner required to manage the service across Kent and Medway
  • Activity Programme for Young People aged between 14 – 35 who have suffered from a first episode of psychosis – Kent Only
