Eczema Outreach Support (EOS) will hold its Annual General Meeting on Tuesday 10th October. The meeting will be held online and follow on from the launch on EOS new Wellbeing resources.

All members of EOS have the right to attend the AGM and to vote on resolutions put forward. If you are unable to attend, you can give your vote by proxy on scheduled resolutions by completing the details below.

Please note you can only complete this form once. EOS may check your details against the details we hold in our membership database in order to verify your membership, but your proxy form will not be stored with your membership records. We will only keep your proxy form for as long as we need in order to correctly document decisions at the AGM
