Please be advised of the following requirements in pursuing employment with PAVE:

  • Certain criminal convictions, pending charges, and negative actions automatically disqualify a person from having unsupervised access to vulnerable adults, juveniles, and children. Here is the link to the DSHS Secretary's List of crimes and negative actions.
  • PAVE's contracts have stipulations in working with vulnerable adults, juveniles, and children that require employees to provide proof of full vaccination against COVID-19. Acceptable proof includes one of the following:
    • CDC COVID-19 Vaccination Record Card or photo of the card
    • Documentation of vaccination from a health care provider or electronic health record
    • State Immunization Information System record
    • WA State Certificate of COVID-19 Vaccination from
    • If vaccinated outside the United States, a reasonable equivalent of any of the above.
Personal attestation is not an acceptable form of verification.
