Hello Alameda Homeowner! 


For owners in the City of Alameda's inclusionary housing programs, we have created this online form which allows each household to submit their annual certification of occupancy and upload supporting documents, securely, online. Once you submit your form and documents, they will transfer into our client management system. Please be assured your information is treated as strictly confidential and is never shared beyond Rise Housing Solutions and the City of Alameda. 

 

All Occupancy Certifications must be received by October 11, 2024.

  • A copy of your property insurance policy declarations for 2024 showing the insurance coverage, additional mortgagees/additional insured, address of your home and the name of at least one of the owners on title.
  • A copy of your most recent Utility Bill (i.e. AMP)
  • A copy of a current Driver's License or California-issued personal identification card of  the owner(s) on title.
