This application allows students to join their school’s debate team, attend practices, and compete at monthly weekend competitions through Chicago Debates, which administers 46 debate tournaments a year for CPS Academic Competitions throughout the city. Note that there are 4 sections of this application - please click NEXT at bottom of the page to advance.

To begin the application and receive a response e-mail confirming your application, please enter a parent/guardian e-mail in the first entry for Email address below.  You can resume completing this application checking the resume form box at the top of the page and providing an email address  and password.

This form gathers information about debaters essential to the operation of of Chicago Debates.   Chicago Debates may share information with the National Association for Urban Debate Leagues (NAUDL). Chicago Debates and NAUDL use information to improve and grow urban debate programs. Chicago Debates and NAUDL may use data to measure student academic progress.  Chicago Debates and NAUDL do not share, sell, rent or trade personally identifiable information with third parties for their promotional purposes.  If you have additional questions please consult our posted privacy policy.

Academic Records Release

In order for Chicago Debates to understand the impact of debate on my child’s academic performance and to better serve my child and other students, I consent to the release of my child’s educational records from Chicago Public Schools to Chicago Debates for the 2021-2022 and 2022-2023 school years.  I understand that education records may include: CPS Student ID number, Attendance, Discipline, Grades, Standardized Test Scores, etc. I understand that this is optional and participation in programming is not contingent upon this release.

Media Release

I hereby grant permission to Chicago Debates, or persons contracted by Chicago Debates, to record, film, photograph, tape and otherwise capture and reproduce in any manner my child’s name, voice, likeness and performance. I grant permission to Chicago Debates to publish in print, electronic, or video format my child's likeness, image and/or information obtained during Chicago Debates’ programming, including, but not limited to, participation in debate practices, tournaments and Chicago Debates sponsored events, for public relations, educational, or any other lawful purposes. I further agree that my child's name and identity may be revealed in connection with the likeness or image.  Additionally, Chicago Debates shall have the right and license to use any biographical materials that my child provides in connection with their participation in Chicago Debates programming and tournaments. Furthermore, I agree to release and hold harmless Chicago Debates, the members, trustees, agents, officers, contractors, volunteers, and employees of each from and against any and all claims, demands, actions, complaints, suits or other forms of liability that may arise out of or by reason of or be caused by the use, production, distribution and promotion of the Recordings. I agree that Chicago Debates shall not be obligated to pay me or any third party any sum whatsoever, regardless of the time or method for any future use of the Recordings. All negatives, positives, prints, digital reproductions, and video recordings shall be the property of Chicago Debates. I release all claims against Chicago Debates with respect to copyright ownership and publication including any claim for compensation related to use of the materials.

Per the consent boxes checked, I agree to the Media Release. I understand that this is optional and participation in programming is not contingent upon this release.

Zoom Opt-Out Waiver

Chicago Public Schools is committed to providing remote learning opportunities for students. Google Meets and Google Live Stream is the District’s secure, integrated, and recommended standard for remote learning during the COVID-19 school closures. However, in an effort to accommodate the needs of the school community during this unprecedented time, schools/teachers may elect to use other tools outside of the District’s endorsed standard, such as Zoom. 

As the parent/guardian, you are responsible for reviewing the Privacy Policy and Terms of Use for the product your school/teachers are using for remote learning purposes. You remain legally responsible for any personal actions taken by your child on this platform and acknowledge that [insert product used, i.e. Zoom] is a third-party platform outside of the control of the Chicago Public Schools. In consideration for your student’s participation in remote learning via the [Zoom] platform, you agree to release, indemnify, and defend the Board of Education of the City of Chicago, the schools, and representatives from any liabilities related to your student’s use of [Zoom].

If you would like to opt your student out of participating in remote learning via [Zoom], please select NO to ZOOM. Selecting No to ZOOM will disqualify your child from participating in virtual Chicago Debates tournaments. Electronic Signatures

By typing in my name below, I certify all information is true and correct to the best of my knowledge. I give full permission for my child to participate and verify the consent options checked on this form. I agree that my typed electronic signature is the legal equivalent of my manual signature on this Agreement and codifies everything agreed to above.
