Service Description

<p>Remove the 11” declining Dogwood at the right front of the home.</p><p>
</p><p>Haul all resultant debris from the property, leaving a stump. </p><p>

TASK: 
The project either has stumpgrinding, planting or some other underground work. Please ask your Arborist if you are unsure of why you have a utility locate request. 
Contractor will call 811 to have the underground public utilities (gas, electric) located and painted before this job is done to prevent any digging to interfere with buried gas lines or electrical wires. Allow 3-5 days after calling to make sure that they come out on time to do the locating. 
Although Contractor is calling in the 811, the Owner agrees to make the Contractor aware of any utilities, public or private, even those that have been located by 811. 811 will typically mark the entire property or area with flags and paint as policy. See Contractor terms covering Underground Utilities.

PERMIT: Boutte Tree will apply for a tree removal permit from the City of Atlanta Arborist Division to remove the following tree(s) based on the following information: 
 Species: Dogwood Diameter in inches (measured at 4.5 feet above ground): 11 Location on property: right front of home Within 5 feet of foundation?no Identifying characteristics: ribbon Reason for Removal: over 50% of canopy is dead 
 NOTE: Applications of 6 more trees require sketch/map of tree locations on the property along with tree identification with either a letter/tree #. This information can be provided in a simple arborist letter or organized list in the system. 
 ** If the tree is located on a neighboring property, we will need written permission from the property owner before a permit can be applied for.
