Thank you for registering to attend our adult classes.  

Before you can participate in a face to face class, we need an accurate record of your contact details, an emergency contact, up-to-date relevant medical information and any necessary consent.

Once this form is completed and your place is confirmed, if we haven't already done so, we will set up a GoCardless payment for the class fees, or send you details of how to make a payment.

All fees are payable in advance of the classes.

We hope all the information we have provided is clear, but if you do have any questions, get in touch.

Participant's Medical Information

Whilst we don't need to know your full medical history, we do need to know about anything that may be relevant to your participation in the classes you've chosen. The information you provide will be shared with the trainers to help them plan the best possible class for everyone taking part. Please include a full list of any allergies you have - whether you consider them relevant or not. Our priority is to ensure everyone enjoys our classes and gets as much from them as possible. Our trainers are skilled at adapting classes to suit the ability of all participants. The more information you provide us with on this form, the better. Thank you for taking the time to complete it fully, it will really help our trainers to prepare and provide a positive experience for everyone.

Acknowledgement of Risk and Code of Conduct agreement

To ensure the safety of all participants and staff, it is your responsibility to ensure you, as the participant, comply with our Code of Conduct and understand the risks involved when participating in circus activities. Please read the following  and confirm your consent before submitting this form.

1. Code of Conduct

Taking part in circus is fun and everyone has the right to be treated fairly, listened to and respected. 

The following lists capture our basic expectations of how all participants will behave and treat each other whilst taking part in a Circusful activity or class.  Therefore, we use the same Code of Conduct for all adult and child participants. 

Please read the following carefully and ask the lead tutor if you have any concerns or questions. Thank you. 

All participants must:
  • Arrive on time and be ready for class, as late arrivals disrupt the workshop.
  • Warm up properly, if you miss the warm-up you may not be allowed to participate.
  • Inform your instructor of any injuries or medical conditions.
  • Always follow the tutors’ instructions.
  • Always use mats/crash mats and the right equipment. 
  • Be aware of your environment. 
  • Take care on uneven flooring and around rigging wires.
  • Let us know if you are having any problems during your class.
  • Cover any open wounds and notify a teacher immediately of all injuries. 
  • Treat all other participants, tutors, staff and building users with respect.
  • Be dressed appropriately in comfortable, easy to move in, clothing such as leggings/ jogging bottoms and a t-shirt.
  • Be mindful and let others know when you are filming in the space. If uploading anything to social media, check with anyone in the background of your footage to ensure they consent to their image being used.
All participants must not:
  • Climb on or touch any equipment when not in class, or without instruction to do so.
  • Distract tutors, staff or other students.
  • Stand or move under aerial equipment, unless using it themselves.
  • Leave the class at any time without informing the tutor.
  • Attempt to teach other participants. 
  • Attempt to climb on the equipment/seating banks, unless explicitly instructed to do so.
  • Attempt to rig equipment or change any of the rigging in the space. 
  • Ask a tutor to get equipment out for you, unless it’s being used during the class.
  • Eat or bring any nuts into the space as we have students with severe allergies.
  • Ever have consumed alcohol or be under the adverse effect of drugs immediately prior to or during a Circusful activity.  
2. Health, safety and risk taking:

Circusful's tutors, staff, performers, freelancers and volunteers work hard to ensure every participant has a positive, safe, experience. We pride ourselves in providing support to all participants to help them learn new skills and to develop creatively, physically, emotionally and socially. However, due to the nature of circus there will always be an element of risk involved.
By completing your name and submitting this form, you are confirming that you understand the risks involved and are aware of the following:

Taking part in circus arts and all other activities that are taught at our premises or by Circusful employees/ freelancers/ volunteers in other locations subjects the participant to a certain level of danger and risk that cannot be eliminated without jeopardising the essential qualities of circus and the arts. You assume your own risk when taking part in all Circusful led activities, regardless of location. 

At no point will Circusful be held liable or responsible for any injury or accident that may occur as a result of participating in a Circusful led activity. 

Circusful tutors/staff/volunteers provide all participants with clear instructions. These must be followed at all times. Failure to follow these instructions carefully may result in increased risk of injury. 

All  participants are responsible for protecting their own health and safety during the session. 

Participants are responsible for confirming they have the appropriate skill and physical capacity to take part and for notifying the tutor/staff/volunteer if they do not understand the instruction they are being given. 

Participants must conduct themselves in a safe manner in the space, adhering to all rules listed in the main registration form, the guidelines listed above and by following the tutors’ instructions, at all times. 

Any participant causing disruption or acting in an unsafe manner may be excluded without a refund from the class.

Circus training incorporates a number of physical styles of movements including running, jumping, vaulting, swinging, hanging and climbing. Dangerous and unplanned movement is actively discouraged. We focus on training students to have good core strength, balance and technique before being introduced to any movement that may involve risk.

Please note: Physical contact with an instructor is often necessary to prevent accident, and/or to demonstrate the moves being taught.
Importantly:

Circusful (the organisers), tutors and associated event organisers accept no responsibility for any damage, loss or injury of any kind, howsoever caused to, or suffered by, any person participating any activity as part of Circusful classes, workshops or events.

Circusful does not tolerate inappropriate behaviour. This includes but is not limited to; violence, vulgar language or bullying of any kind. If this kind of behaviour is witnessed the participant and will be spoken to/contacted at the earliest opportunity. 

Circusful Staff/tutors are not able to assist participants with toileting, dressing/changing or other matters related to personal hygiene etc. If you are concerned this will exclude you from participating in our classes please telephone the office on 028 9023 6007. 

3. Circusful Code of Conduct and Risk Waiver - Declaration

I declare that I have read, understood and accepted all the matters, including all the risks, responsibilities and obligations, to which they refer.  I consider that I am in good health and capable of taking part in this activity. I am doing so voluntarily, and I confirm my consent to take part acknowledging the risks. I acknowledge that if anyone is hurt or property is damaged during my participation in any Circusful activity, I may be found to have waived my right to maintain a legal case against Circusful on the basis of any claim from which I have released them herein.  

By completing this form you are confirming that you have read the liability waiver, understand the risks and agree to comply with our Code of Conduct.
