Evidence of brain injury

In order to provide you with the Headway Brain Injury Identity Card, we require confirmation of your brain injury from your local Headway group or branch, or a clinical professional.

There are two ways to do this.

1 - Upload your evidence

You can upload your evidence via this form. This is the quickest option as it will allow us to start processing your application straight away.

2 - Allow Headway to contact someone for you

If you cannot upload proof of your brain injury, we can request it on your behalf.

Please provide the contact details for one of the following:
  • Your local Headway
  • Your GP
  • The hospital or healthcare professional who treated you
Evidence must include:
  • Your full name
  • Date of birth
  • Medical diagnosis
  • Medical professional’s signature
  • Letter head to show where the document was produced (e.g. from the NHS or a private hospital)
Please note we are unable to accept documents which do not show all of the above information.
If you are an active member of a local Headway Charity or volunteer-led branch:

Tell us which local Headway you attend, and we will contact them to request confirmation of your brain injury.

If you are not a member of a local Headway Charity or volunteer-led branch:

You can ask us to contact your GP or another healthcare professional instead.

Before we can do this, we need your permission. Please download, complete and return this consent form via email to idcard@headway.org.uk. Once we receive your signed consent form, we can contact your healthcare professional and begin processing your application.

After you submit this form, we will send you a link to a short survey asking for your feedback on the Brain Injury Identity Card application process. This survey is entirely optional, but your answers will help us to ensure the process is working for all applicants.

We’d also like to follow up in six months’ time with a survey about the card holder’s experience of using the Brain Injury Identity Card. This important research allows us to better understand how the card is being used, and make improvements to the scheme in future.
