2. What is the estimated number of students in your school?

This grant is designed to serve a cohort of up to a maximum of 180 students

chosen from one or two grade levels. While this cohort of students will be the primary recipients of grant programming, the team can gift parts of the grant to other grade levels or choose assemblies for wider participation in the school.

If your school is small or mid-size, you can include other grade levels as long as the

cohort of students does not exceed 180 students. If your school is large, select one grade level only.

Examples:

Elementary School: three sections of 3rd grade (90 students total) and two sections of

5th grade (60 students total). Total students part of the cohort :150. Teachers on the grant team should have a representative from both grade levels on the team.

Middle/High School: four sections of 7th grade (120 students total) or groups of selected students within a grade level

11. Will the entire teacher team named in this application be able to commit to attending at least four grant planning sessions through the year? The majority of the planning happens in the fall. Initial 2-hour Kick-Off Planning Meeting happens in August with lunch provided; other one-hour planning meetings are scheduled in late September, November and January. Please note any scheduling challenges.

Identify your team

Create a team of 3-5 teachers with a combination of teachers representing: Science, Social Science, Language Arts, Visual Arts, Music, Theatre, Dance and/or another subject area. Teams can include Teacher Librarians, Instructional Coaches, Social Emotional Learning Specialists etc...

Elementary Schools: assign a discipline that most suits each teacher to oversee programs in that area.

Middle Schools and High Schools: indicate your subject area
