This document is intended to address the McKinney-Vento Assistance Act. Your answers will help determine the eligibility for services and expedite enrollment for your child/children. If your family is living in any of these situations you might be eligible for extra support:

Staying in a shelter
Sharing a house with others because of loss of housing or economic hardship
Living in a car, park, or campground
Living in housing without water, electricity, or heat
Living in a garage without access to a kitchen or bathroom
Living in a hotel or motel temporarily because of loss of housing or economic hardship

A Caregiver Affidavit allows a Caregiver to enroll a child in school and consent to a child’s medical care. A Caregiver’s Affidavit does not take away or suspend parental rights. A parent or guardian can overrule any of the Caregiver’s decisions, unless the parent or guardian’s wishes jeopardize the child’s health, life, or safety. This is a court document.
