Talent Release Form

I give permission to The Duke of Edinburgh’s International Award – Australia (ABN 70 114 267 195) (The Duke of Ed) to:

1. take photographs, videos and sound recordings of me, including of performances, stories and quotes by me, while I am engaged in Duke of Ed Award activities (Materials), including my personal information contained in such Materials
2. use in the manner set out below images, videos, stories, quotes and other content supplied by me directly to Duke of Ed or indirectly via social media channels where I tag Duke of Ed (Other Content), including my personal information contained in such Other Content
3. use (including by reproducing, communicating to the public, causing to be seen or heard in public, publishing or adapting) the Materials or Other Content, including my personal information contained in such Materials and Other Content, in any traditional (eg print or television) or modern media (eg internet and social media) for the following purposes:
1. promotion of and reporting on The Duke of Ed or its state/territory/overseas affiliates and their related objectives and activities, including in brochures, posters, web pages, educational content, promotional displays and advertising material; or
2. any other purpose as agreed by Duke of Ed and me
4. choose to use my name or no name in relation to images of me in the Materials or Other Content, except if I have indicated below I do not wish my name to be used with such images
5. credit me as the author of Materials by me or Other Content, except if I have indicated below I do not wish to be credited as the author
6. allow other persons affiliated with The Duke of Ed (eg contractors to The Duke of Ed or its state/territory/overseas affiliates) to create or use the Materials or Other Content for the purposes set out in, and in accordance with this, consent;

I acknowledge that:

1. the Duke of Ed will not pay me for providing this consent, or for the use of Materials or Other Content covered by this consent; and
2. I will not be given an opportunity to inspect or approve the Materials or any use of the Materials or Other Content provided such Materials or Other Content are used in accordance with this consent.

I confirm that any stories or quotes or other content by me, or provided by me, that are included in the Materials or Other Content are:

1. original and not copied from someone else; or
2. used and provided by me with all necessary third party permissions for the uses set out in this consent.

By providing my personal information, I agree that it will be used and disclosed by The Duke of Ed in accordance with this consent and The Duke of Ed privacy policy available on its website.

I am the parent/guardian to the Participant identified in this form and I provide consent to this Participant to give the consent as described above.
