INTRODUCTION
Allen County has been awarded $39,349 through Phase 41 of the Emergency Food and Shelter Program (EFSP).  

Allen County agencies (non-profit, faith-based or government) are invited to apply for these EFSP Funds by completing the following application, including supporting documents.  

DEADLINE FOR APPLICATION:   5:00 pm, Tuesday, February 20, 2024.

This application will be automatically submitted to the United Way of Greater Lima office after you take the following steps:

  1. Once you start the report – you will be given the opportunity to “save and resume” later.  Please keep a record of your password.   
  2. When submitting the form – There are two steps to compete your submission.  You will first click “submit” at the bottom of this form.  Once submitted, you will be given the opportunity to review and/or correct, and print your final application.  Once you are satisfied that your application is complete, you will need to click “confirm" again at the bottom of the page.  You will know it was submitted successfully when a "Thank You" message appears on the page.  
The "Local Board" will review all applications in preparation for the:   Local Board meeting scheduled for February 22, 2024.  

Agencies applying will be granted 10 minutes at that meeting to explain their proposed plan and answer questions, starting at 3 pm  The Local Board will subsequently notify all applicants of the proposed Local Board Plan for allocation of the funds.  

Please note: 
  1)  Regardless of recommendations made by the Local Board, no organization will receive funds from the current Phase until all EFSP requirements have been met for the previous Phase/Phases.
  2)  The Local Board is required to ensure that no duplication of payment occurs within the expenditure categories of rent/mortgage assistance or utility assistance.  That is, multiple Local Recipient Organizations (LROs) may not provide assistance to the same client for the same service.

The Local Board will submit their "Local Board Plan" to the National Board

REQUIRED DOCUMENTATION
The EFSP Program requires specific documentation from its' "Local Recipient Organizations" (LROs).  Please upload the MOST RECENT version of all the following documentation.  In addition, please follow these instructions for uploading successfully.  
  • All attachments in pdf format should have portrait orientation and be sized to 8 1/2 x 11 only.
  • The file size should not exceed 35 MB.
  • If a document consists of multiple pages, it needs to be uploaded in one file.  Do not attempt to upload one page at a time on any question requiring upload.
  • The file extensions accepted by the system are pdf, doc, docx, xls, xlsx, ppt., pptx. (PDF is PREFERRED.)
