Program Eligibility Questionnaire
Tree Trust Summer Youth Employment Program is an employment training program serving eligible young people. Please THOROUGHLY read the questions below and answer honestly to assess your eligibility. 

SYEP program runs between June 20- August 18, 2023.  


Tree Trust is a nonprofit organization that provides employment training to youth and young adults. The YouthBuild Programs serves eligible young adults. Check the appropriate answers below to assess your eligibility.
Tree Trust is a nonprofit organization that provides employment training to youth and young adults. The Branches Program serves eligible young adults. Check the appropriate answers below to assess your eligibility.
Tree Trust is a nonprofit organization that provides employment training to youth and young adults. The Career Pathways Program serves eligible youth and young adults. Check the appropriate answers below to assess your eligibility.

To participate in the program, you must:

  • Live in Suburban Hennepin County (Preferred)

  • Be 14 through 24 years old (priority given to 18-24 years old)

  • Meet income guidelines or not currently enrolled in school or have a documented disability (IEP/504 plan)

I give permission for the Department of Employment and Economic (DEED) unemployment Insurance Division (UI) to release information in the way described on this form. I agree to let DEED UI release these data to the Workforce Center System for up to three years. I understand that YouthBuild program staff needs to collect the data in order to determine outcomes for YouthBuild programs. I understand that, whether or not these data are released to YouthBuild Program staff will not affect my participation in the YouthBuild program.  *If you are under 18 years of age or not your own legal guardian your legal guardian will verify this consent in the e-signature forms that will come after this application received.
