All participants under the age of 18 are required to submit this Parent/Guardian Liability Release form before participating in Tahoe Area Mountain Biking Association ("TAMBA") volunteer projects.

I certify that My child/ward (children/wards) listed above has my permission to participate in volunteer activities with TAMBA. Volunteer activities may include, but are not limited to, trail construction and/or maintenance projects. With my signature, I acknowledge that TAMBA volunteer activities involve natural and human caused risks that may result in injury or death. I assume all liability in the event that my child/ward (children/wards) is injured or that property belonging to me or my child/ward (children/wards) is damaged while engaged in volunteer activities with TAMBA. I also hereby agree to indemnify and hold harmless TAMBA and any of its officers, employees, agents or other volunteers from any injury, damage, claim, or loss of any kind sustained by me or my child/ward (children/wards) or any of the officers, employees, agents or other volunteers of TAMBA

By submitting this form I acknowledge that my child/ward (children/wards) will not receive any compensation for his/her service and, as a volunteer, is NOT considered an employee of TAMBA or the land manager/owner upon whose property TAMBA volunteer activities take place for any purpose. I also acknowledge that all photos or videos of my child/ward (children/wards) captured during volunteer activities constitute property of TAMBA and the land managing agencies. I hereby expressly give my consent that any such photos or videos of me and/or my children/wards may be used in TAMBA publications or promotional materials and are not subject to copyright laws.

I do hereby certify that to the best of my knowledge my child/ward (children/wards) is in good health. If it should become necessary for my child/ward (children/wards) to receive medical treatment and I am not available, I hereby give my permission for TAMBA leaders and/or a licensed healthcare provider to administer the medical treatment s/he deems necessary, including hospitalization to save the life of my child/ward (children/wards) or to relieve pain and suffering. I understand that every reasonable effort will be made to contact me in the case of such an event.
