Thank you for completing the lesson registration request form. Space is limited and based on appropriate resources.

Physician’s statements must be completed within one year of the last form submitted.  You may print a form  from the website for the participant's doctor to complete, sign, date. Completed forms should be returned to the program director.

The LTR Program Director will contact you with a final schedule. Invoices for program service fees will be sent by email (check your SPAM folder) and due prior to the first day of the session.
