Application & Report Extension Request Form

You can request a deadline extension for your grant application or grant report by submitting this online Extension Request Form to Mid-America Arts Alliance (M-AAA). An extension gives you extra time to finish your application or reports.

This form must be completed 5 days before the application or report deadline with justification for an extension. All requests are considered on a case-by-case basis and approval is not guaranteed.

Upon review of this form, M-AAA will notify you if your extension has been approved or not approved within 48 hours.
