GO Project Overview | Descripción General Del GO Project | GO 项目简介
The GO Project shapes the futures of New York City public school children by providing critical academic, social and emotional support starting in the early elementary years. GO provides year-round educational and family support services to children who are performing below grade level and equips them with the confidence and skills needed to succeed at school, at home and in life. After completing GO Project, students enter high school prepared for the academic challenges that lie ahead of them and confident that they are able to succeed academically, socially and emotionally.

El GO Project ayuda a formar el futuro de los alumnos de las escuelas públicas de la Ciudad de Nueva York, proporcionando apoyo crítico en lo académico, en lo social y en lo emocional a partir de los primeros años de escuela primaria. Requirements for Participation | Requisitos Para Participación | 要求
A year-round & year-to-year commitment is required for participation in the GO Project. This includes participation in GO Summer, a five-week Monday-Friday program and GO School, a seven-month, Saturday morning academic program during the school year. GO Project also offers additional weekday remote literacy support to select students who are participating in the year-round programming.

We are currently accepting applications for the next program cycle. In order to be considered, a parent/guardian must attend a Family Information Session (remote by Zoom).

ACADEMIC RELEASE
By checking yes below, you are giving GO Project permission to have access to any relevant academic information that will help us create an individualized academic plan for your child in our comprehensive year-round programming. Once enrolled in the GO Project, this information can be accessed each year for the duration of the commitment to GO Project programming. In order for GO Project to provide the best services for your children, it is important for us to have access to your child’s school records including information about enrollment, grades, test scores, OSIS numbers (Department of Education numbers), Individualized Educational Plans, and attendance information. Additionally, we will ask that your child’s public school teacher complete a student evaluation form documenting their strengths and areas for development.  Individualized Educational Plans, student evaluations, and report cards will be shared with your child's GO Project teacher.  By checking yes below, you give GO Project your permission to access the afore-described records.  Should you change your mind and decide that you do not want us to access your child’s records, you may contact us and withdraw your consent at any time.

PERMISSION TO PUBLISH PHOTOGRAPHS AND OTHER INFORMATION
I hereby authorize GO Project to take and use black and white, color, slide, and/or video photographs of my child or me for use in all media including but not exclusive of newspapers, fundraising materials, and/or social medial for any purpose that is consistent with the stated goals of the GO Project. I understand and agree that neither I nor my child will receive any financial compensation for the publication of any photos and information.

Should you change your mind and decide that you do not want us to use photographs or videos of your child, you may contact us and withdraw your consent at any time.

HELLO INSIGHT OPT OUT FORM   
Hello Insight is an online platform that enables the GO Project to measure our students social and emotional learning.
 
Because your child participates in the GO Project program, they will be invited to fill out an online or paper survey at the beginning and end of each program cycle. These surveys measure the impact of the program on your child’s development and capture your child’s experiences within the program, allowing us to make critical improvements to meet their needs. All youth involved in the GO Project program will be invited to participate in this survey.
 
How will your child participate in the survey?
The surveys take about 20 minutes to fill out and will typically occur during normal program hours when the program meets. Your child will be invited to complete the survey in a group setting online or on paper.

Participation is completely voluntary. You and your child have the right to terminate their participation at any time or to refuse their participation entirely without jeopardy to their status in the program. None of the activities will interfere with your child’s instructional time or affect their academic grades.
 
What are the benefits to taking the survey?
Your child will become more reflective about their skills and behaviors. Reflection is a key developmental skill that has been shown to lead to a wide-range of benefits for young people.
Additionally, as the adult staff learn more about your child’s interests and experiences in the program, they will be able to tailor activities to meet their needs.
 
Are there any risks or discomforts in taking the survey?
Risks are minimal to none for involvement in this study. However, your child may feel emotionally uneasy when asked to make judgments about themself and the program.
 
How is my child’s confidentiality protected?
All information obtained from your child on the survey will be kept confidential and will be used by staff to increase the quality of the program. Program managers, and possibly external evaluators, will also have access to the data so that they can continue to improve the program and support your child’s individual needs.
 
All data are gathered and analyzed through an online platform called Hello Insight. Your child’s name will not be associated with data in Hello Insight. Data within this platform are kept confidential in a secure database. Hello Insight will never use your child’s name for any reason related to this data, and all data displayed through this system will highlight combined or group results.

Debido a que su hijo participa en el programa GO Project, se le invitará a completar una encuesta al principio y al final de cada ciclo del programa. Estas encuestas miden el impacto del programa en el desarrollo de su hijo y capturan las experiencias de su hijo dentro del programa, lo que nos permite satisfacer sus necesidades. Todos los jóvenes involucrados en el programa GO Project serán invitados a participar en esta encuesta. 
  
¿Cómo participará su hijo en la encuesta?

Los administradores del programa, y posiblemente los evaluadores externos, también tendrán acceso a los datos para que puedan continuar mejorando el programa y respaldar las necesidades individuales de su hijo/a. 
 
Todos los datos se analizan a través de una plataforma en línea llamada Hello Insight. El nombre de su hijo/a no se asociará con datos en Hello Insight. Los datos dentro de esta plataforma se mantienen confidenciales en una base de datos segura. Hello Insight nunca usará el nombre de su hijo/a por ningún motivo relacionado con estos datos.
