CARVER CHRISTIAN ACADEMY APPLICATION 2024-2025

Welcome to Carver Christian Academy's Online Application. 

We are so excited that you are interested in sending your child to Carver Christian Academy (CCA). 

Carver Christian Academy exists to assist parents in the endeavor to help our students "increase in wisdom and in stature and in favor with God and man". (Luke 2:52). It is our mission to spiritually and academically foster a biblical worldview that will challenge, shape, and inspire our learners to think critically influencing the world for Christ.  Learners will achieve academic excellence, embody obedience, respect, and responsibility as they follow God's purposes for their lives.

Carver Christian Academy will begin the school year in person on August 12, 2024. The school is located on Urban Youth Impact's campus at 2823 N. Australian Avenue. We are committed to helping parents cover the costs of the tuition and fees through scholarship opportunities, including Florida state scholarship programs and additional scholarships offered through Urban Youth Impact. 

Our school zone includes the Tamarind and Windsor Avenue neighborhoods.
Students who live from:
Banyan Boulevard to 45th Street, and east of Mangonia Circle to west of US-1 . Transportation is limited; however, we do offer free transportation as needed. The school day begins promptly at 8:15 am. Students must arrive between 8:00 am- 8:15 am. Breakfast and Lunch will be provided. School hours are 8:15 am- 2:45 pm. If your child requires aftercare, please contact Daniella Darling. We have reserved a limited number of spots for children to attend Urban Youth Impact's after-school programming that will run from 2:45 pm- 5:00 pm.
Please submit the following items to the Admissions Office as soon as possible to ensure timely processing of your application:
  • Copy of Birth Certificate
  • Copy of Up-To-Date Florida Health Examination Form (DH3040)
  • Copy of Up-To-Date Florida Certificate of Immunization Form (DH680)

In the event of an accident, injury, or illness that requires emergency medical treatment, your child's medical expenses will be covered by your primary insurance carrier.  If your child does not currently have health insurance coverage, the Parent/Guardian is fully responsible for any medical expenses incurred.

If transportation is needed from CCA, the vans will only travel within the following radius: 
   

North: 45th Street

East: Broadway/Spruce/Rosemary

West: Australian/Mangonia Circle/ 7th Street

South: 3rd Street

The van will pick up students from the student's address indicated in the application within the defined limits on a case-by-case basis.

If you have these documents; Immunization Records, Physical Exam and Birth Certificate,  please submit them below. If not, we must have them by July 29, 2024, or your child will not be able attend on his/her first day of school.

Financial Aid 

I acknowledge and agree that I must participate in any scholarship opportunities that are available to me, and though these funds will only cover a portion of the cost, Urban Youth Impact & Carver Christian Academy will fundraise to cover the additional costs, as long as the students meets the academic and behavioral expectations. 

Step-Up Scholarship Application Required Documents 

Our school has partnered with Step-Up to offer Financial Aid to our students. You will need to apply with them to be awarded scholarship funds for our school. Below you will find the documents you will need when submitting an application for your child. 

1. Proof of Florida Residency:
-Valid Drivers License or
-Recent Utility Bill (within the last two months)
2. Social Security Number:(for you and the student)
3. Birth Certificate of Student
4. Proof of Income:
-Pay stubs from the last 30-days or
-Unemployment Benefits or
-Social Security Benefit's or
-Child Support Benefits

  • Residence in the Tamarind Avenue Corridor
  • Completed Application
  • Submission of all Health Records
  • Submission of Step-Up Scholarship Application
In order to be considered for enrollment all the terms above need to be complete. You will be notified of your child’s status once we have all your information in our secure database.
