Personal Information

The next three questions are used to track the demographics of our volunteers. Providing this information is optional.

Wild Montana Volunteer Acknowledgement and Assumption of Risk Agreement

Wild Montana makes efforts to keep volunteers safe by providing safety gear, instruction, and experienced leaders in the field. However, trail projects have inherent risks that are not eliminated even after intentional mitigation. Each volunteer must take responsibility for their decisions as well as understand and accept the potential dangers and risks involved with volunteering. Read the Volunteer Acknowledgement and Assumption of Risk Agreement and let us know you agree to the terms stated. Please click on the document link if you haven't already to review.
BY CHECKING THE BOX ABOVE, YOU MAY BE WAIVING YOUR LEGAL RIGHT TO A JURY TRIAL TO HOLD WILD MONTANA LEGALLY RESPONSIBLE FOR ANY INJURIES OR DAMAGES RESULTING FROM RISKS INHERENT IN THE RECREATIONAL OPPORTUNITY OR FOR ANY INJURIES OR DAMAGES YOU MAY SUFFER DUE TO WILD MONTANA’S ORDINARY NEGLIGENCE THAT ARE THE RESULT OF ITS FAILURE TO EXERCISE REASONABLE CARE.
Under 18 Years Old?

Wild Montana requires that any individual under 18 years old be accompanied by a responsible adult. A responsible adult is defined as a parent, legal guardian, or trusted adult of the minor; Wild Montana also requires them to agree to the Volunteer Acknowledgement and Assumption of Risks Agreement and check the box below.

Wild Montana Risk Hazard Assessment

Wild Montana makes efforts to ensure that you are aware of the risks associated with volunteering on a Volunteer Trail Crew project. It is imperative that as a willing participant you are aware of the inherent risks associated with the work, the likelihood of these risks occurring, and how to best mitigate them. Each volunteer must take responsibility for their decisions as well as understand and accept the potential dangers and risks involved with volunteering. Read the Risk Hazard Assessment Form provided by the United States Forest Service, and let us know you agree to the terms stated. Please click the document link if you haven’t already to review.

Emergency Contact

Please enter info for someone that will NOT be on the volunteer project with you. Make sure this person knows they are listed as your emergency contact and knows when you will be on the project.

Medical Information

Volunteers must self-administer medications and inform project leaders where medications are stored in event of emergency. Contact Wild Montana if any medical or health condition changes before the start of selected project.

All medical information is kept confidential. Please understand that although Wild Montana staff may review your health information and accept your registration, ultimately, it is the participant’s (and parent/guardians of minors) responsibility, in consultation with their physician, to determine if Wild Montana activities are an appropriate match.

Dietary Considerations
Meal planning and cooking for a big group is a difficult task. We will do our best to accommodate your needs. If you have especially stringent restrictions we may ask you provide your own food.
