Please note that this is a three day-long camp. All attendees are expected to fully attend all three days. There will be a requirement for the parent/guardian to ensure the attendee has ability to access transportation to and from the camp each day.

Student Consent to Participate

I hereby consent and provide permission, as part of my participation in CAREERS programs, events, and activities, to be photographed, recorded by video and other electronic means, and be interviewed for usage in CAREERS’ promotional material or other media, including video productions, annual reports, website and brochure graphics, electronic media, newspaper articles or ads, CAREERS student customer satisfaction surveys and/or approved CAREERS program surveys, and event programs. I understand that this material will remain as the property of CAREERS: The Next Generation.

Liability and Assumption of Risk 

ASSUMPTION OF RISK AND RELEASE, student

I am aware that I am traveling each day on March 23, 24 and 25, 2024 to participate in off-campus activities for the Young Women in Trades and Technologies Spring Camp (“CAREERS Spring Camp"). I am fully aware of the risks and dangers involved in my participation in the CAREERS Spring Camp. I am aware, although remote, that unanticipated and unexpected events may occur that may result in death, personal injury, property damage or other loss to me. I hereby freely and voluntarily accept and assume any and all risks of injury or damage that may be sustained by me in connection with the CAREERS Spring Camp.

The Board of Directors of CAREERS: The Next Generation, its officers, contractors, employees and volunteers (collectively referred to as the “Releasees”) are not responsible for any injury, loss, claim, liability or damage of any kind sustained by me while participating in the CAREERS Spring Camp, whether caused by the negligence of the Releasees or otherwise.

 

RELEASE OF LIABILITY, WAIVER OF CLAIMS AND INDEMNITY AGREEMENT

In consideration of CAREERS: THE NEXT GENERATION allowing me to participate in CAREERS Spring Camp, including but not limited to any associated activities, travel, transportation and notwithstanding any insurance or, if applicable, workers' compensation coverage maintained by Alberta Education, I hereby agree to:

  1. ASSUME AND ACCEPT ANY AND ALL RISKS arising out of, associated with or related to my participation in the CAREERS Spring Camp, whether caused by the negligence of the Releasees or otherwise;
  2. BE SOLELY RESPONSIBLE FOR ANY AND ALL INJURY, LOSS, LIABILITY OR DAMAGE which I might sustain participating in the CAREERS Spring Camp, whether caused by the negligence of the Releasees or otherwise;
  3. WAIVE ANY AND ALL CLAIMS against the Releasees and forever and discharge the Releasees from any claim, loss, injury, liability, cost, damage or expense I may suffer, including without limitation, personal injury or death, whether caused by the Releasees or otherwise;
  4. HOLD HARMLESS AND INDEMNIFY the Releasees from any and all liability for any damage or loss to the property of or death or personal injury to any third party resulting from my participation in the CAREERS Spring Camp.
  5. INDEMNIFY AND HOLD HARMLESS the Releasees from any and all claims, demands, actions, causes of action and costs made against the Releasees which might arise out of my participation in the CAREERS Spring Camp.

 

ACKNOWLEDGEMENT 


certify that I have read this document and I fully understand its content. I am aware that this is a release of liability and a contract and I sign it of my own free will. In return, I will participate in the Young Women in Trades and Technologies Spring Camp activities. I will also make every effort to obey safety precautions as listed in writing and as explained to me verbally. I will ask for clarification when needed.








Parent Consent

Your daughter is completing the Young Women in Trades and Technologies Spring Camp application to participate in the Calgary Region program. As the Spring Camp provides real job experiences, students are treated like young adults, expected to show maturity and sound judgment. This is an excellent opportunity for your daughter to gain confidence in making an educated career choice from her experience with the program. Please check the boxes below, sign and date.
