I agree to provide any documentation needed to verify this information, at the request of the Mass Save Sponsors or their implementation vendor(s).
I understand that any other income-based Mass Save offers I may wish to pursue will require income verification and that this attestation does not constitute a statement of eligibility for any other incentives.
I also understand I may be required to repay any incentives I receive if the information I provided about my household income is false or inaccurate.
