We are carrying out some research to find out more about the experiences of parents using the CMS, and to find out who is not using the CMS and why. We would like to speak to people in small groups or on a 1-1 basis. If you would like to take part, please fill out this form, so we can get some initial information about you and your situation.

Please note that by completing this form you are not obliged to take part in the research. We will send you more detailed information about the research and how we will use your information, and you can decide whether to take part or not.

Because we want to speak to people with a range of experiences, we may not be able to include everyone who registers an interest in a focus group or interview. As part of this research we will also be conducting a survey, so you will also have the opportunity to share your experiences through the survey.

Your data will be stored and managed according to our privacy policy. Your responses will be treated confidentially, and your contact details will only be used to contact you in relation to this research.
