SYO Orchestral Musicianship Absence Form

Attendance at SYO your Orchestral Musicianship classes is a vital part of your SYO membership. Musicians are expected to arrange their schedules so that they can attend all activities as per the SYO Participant Code of Conduct. If you are unable to attend a scheduled Orchestral Musicianship class, please submit a form for each day you will be absent. This includes late arrivals and early departures.

If you need to attend a class other than your assigned time for a single date please select the 'Changed Class Time' option

If you need to change your class time for the rest of the year

please select the 'Ongoing clash' option

The SYO Musicians and Training team will receive your form and contact you if there are any issues with your request. If you do not hear from SYO, please consider your absence accepted and noted.

Please fill in one class per form unless you are filling in this form notifying us of an ongoing clash. In that case, SYO staff will contact you to verify that you have a prior commitment and change your attendance records for you for the remainder of the year.

If arriving late or departing early please clearly state your estimated arrival or departure time e.g. Arriving at 2:30pm due to school athletics carnival.
