SYO Orchestral Musicianship Absence Form

Please see below for orchestral musicianship attendance expectations. All musicians who are part of SYO are expected to attend all orchestra activities as per the SYO Participant Code of Conduct.

1. If any SYO musician is unable to attend their scheduled musicianship class for any reason they must fill out an absence form.

2. If you need to attend a class other than your assigned time for one week only due to an unexpected clash this is acceptable. You need to select 'request for changed class time' on the absence form and specify what class time you would like to change to in the 'Reason for Absence/Changed Class time' box.

3. If you need to change your class time the rest of the year please contact syo staff directly at info@syo.com.au to discuss this further.

4. Acceptable reasons for absence/changed class time include

- Illness

- Prior commitments (such as a sport tournament, or after school rehearsal)
- Unavoidable ongoing clash with another commitment. Please select 'Ongoing clash' in your reason for absence if this is the case and SYO staff will contact you to verify your reason and then change your attendance records to indicate you are unable to attend for the remainder of the year.

SYO management will receive your online notification and contact you if it requires further discussion. If you do not hear from SYO, please consider your absence accepted and noted.

Please fill in one class per form unless you are filling in this form notifying us of an ongoing clash. In that case, SYO staff will contact you to verify that you have a prior commitment and change your attendance records for you for the remainder of the year.

Please make sure you have selected the correct date before submitting your form. Absences must be submitted at least one day before any activity.

If arriving late or departing early please clearly state your estimated arrival or departure time e.g. Arriving at 2:30pm due to school athletics carnival.
