The BTS Center will be documenting this event with photography, videography and audio recordings, which may be used for promotional and educational purposes.

I grant The BTS Center permission to use such materials that include photographic or video /audio recordings of me.I do not grant such permission.

Accident Waiver and Release of Liability

I hereby assume the risks of participating in any and all activities associated with Claiming Your Call for a Climate-Changed World, June 20-22, 2024, at Schoodic Institute in Winter Harbor, Maine. I acknowledge that participating in this gathering may pose some risk of personal injury and that I undertake and assume this risk for myself. I acknowledge that this Accident Waiver and Release of Liability Form will be upheld by the participants, hosts, sponsors, and organizers of the activity, and that it will govern my actions and responsibilities at said activity. I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:

A. Waive, Release, and Discharge The BTS Center, Creation Justice Ministries, Anabaptist Climate Collaborative, and Schoodic Institute, and/or their directors, officers, employees, volunteers, representatives, and agents, and the activity holders, sponsors, and volunteers, from any and all liability, for my death, disability, personal injury, property damage, property theft, actions of any kind which may hereafter occur to me including my traveling to and from this activity, or claims of any nature which may hereafter accrue to me and my estate as a direct or indirect result of my participation in the above referenced activity;

B. Indemnify and Hold Harmless the entities or persons mentioned in this waiver from and against any and all liabilities or claims of any nature, including all costs, expenses, and attorneys' fees, which in any manner result from participating in this activity or event.

I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during this activity.

I acknowledge that this Accident Waiver and Release of Liability Form shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law. I agree that if any portion is held invalid, the remainder will continue in full legal force and effect.

I, the undersigned participant, affirm that I am at least 18 years of age and am signing this agreement of my own free will. I understand that by signing this form I am giving up legal rights and/or remedies which may otherwise be available to me regarding any losses I may sustain as a result of my participation. I have read this form and fully understand that it is a release of liability and a contract.

Research Guidelines

In addition to offering the “Claiming Your Call Retreat and Community of Practice” as an opportunity for discernment, formation and praxis for congregations, The BTS Center looks forward to learning and drawing conclusions through surveys, participant observations, interviews, and site visits, in ways that will benefit Claiming Your Call community participants, The BTS Center, and future potential audiences and constituencies, including other local faith communities.

The non-anonymized results of this research will be shared only with members of The BTS Center research team. Any information shared with wider audiences will be anonymized beforehand, respecting both community guidelines and confidentiality.
