Immunization Form 3231 Submissions



Georgia law (O.C.G.A. ยง 20-2-771) requires a certificate of immunization on file for each child in attendance in any child care facility in Georgia.


The only accepted certificate is the DPH Form 3231. This form must be completed, signed, and dated by your pediatrician. No other forms of documentation can be accepted.  


If you are new to Georgia, all out of state immunizations must be transcribed on the Georgia DPH Form 3231 by a physician for the form to be accepted.

Religious Exemption to Immunizations


The Affidavit of Religious Objection to Immunization form should be filed with IPCP instead of the DPH Form 3231. This Affidavit does not expire. 

**IPCP cannot accept the DPH Refusal to Vaccinate Form. According to the Georgia Department of Education, there are only two exemptions - Medical or Religious exemptions.**


Medical Exemption to Immunizations
A medical exemption must be marked on the Georgia Immunization Certificate (Form 3231). A letter from a physician, Advanced Practice Registered Nurse (APRN) or physician assistant (PA) attached to the certificate will not be accepted as a medical exemption. It must be marked on the certificate. A medical exemption requires an annual review. Your physician must provide this every year with the medical exemption marked and the new expiration date.
