Volunteer Code of Conduct 


1. I will make the safety and well-being of one·n·ten youth the basis of my decisions and actions. 

2. I understand that for my safety and the safety of one·n·ten youth, I will avoid being alone with a single youth where others cannot observe me. 

3. I will use positive techniques of guidance, including redirections, positive reinforcement, and encouragement rather than unhealthy competition, comparison, and/or criticism. 

4. I understand and agree to treat all youth with respect, consideration, and care and treat all youth equally regardless of gender identity, sex, sexual orientation, age, ability, ethnicity, race, spirituality, or culture. 

5. I will portray positive role modeling for youth by maintaining an attitude of respect, loyalty, patience, and maturity. 

6.  It is strongly recommended that there are no personal relationships or social contact* beyond the program’s scope (e.g. one-on-one friendships or social interactions outside of approved programmatic time) with youth between the ages of 18 to 24. *Social contact includes but is not limited to: meeting youth for coffee, movies, recreational events, dining, dating, or providing transportation. All communication with youth shall occur at official one·n·ten events or programs. Personal contact information (phone number, social media accounts, email address, home address, etc.) will not be shared with youth participants of one·n·ten. 

7. There may be times that you see one·n·ten youth or volunteers in public. Since youth may not be "out" please do not approach youth to say hello, unless they say hello to you first. An exchange of hello is appropriate, but no further contact should occur. 

8. I understand that one·n·ten prohibits volunteers from using tobacco, alcohol, and/or drugs (the exception being medication prescribed by a doctor, excluding medical marijuana) while on duty. 


*Any person 11-24 years old met through one·n·ten sponsored activities is considered a youth of one·n·ten 


I have read and fully understand the one·n·ten Volunteer Code of Conduct and agree to abide by the above rules and guidelines. I understand that if the above rules and guidelines have not been adhered to, I may be suspended or removed as a volunteer.

Photo Release Form 

 Read this document carefully before signing below. 

 

I (herein known as one·n·ten Volunteer) am an active participant of the one·n·ten Youth Center or Satellite location, a social component youth service of 1N10 Inc. 


I hereby grant one·n·ten, Inc. and one·n·ten staff permission to use my likeness in a photograph or video in any and all of their publications and promotional materials, including internet and website entries, without payment or any other consideration. I understand and agree that these materials will become the property of 1N10 Inc. and will not be returned. 


I, hereby irrevocably authorize the 1N10 Inc. to edit, alter, copy, exhibit, publish or distribute these photos or videos for purposes of publicizing one·n·ten programs or for any other lawful purpose. In addition, I waive the right to inspect or approve the finished product, including a written or electronic copy, wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising from or related to the use of the photographs or videos.  


I Hereby hold harmless and release and forever discharge one·n·ten from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization. I agree to indemnify and hold one·n·ten harmless for any and all losses, claims, expenses, suits, costs, demands, and damages, or liabilities arising out of the photographed or videotaped activities.

Consent to Treatment and Release of Liability

I (herein known as a one·n·ten Volunteer), understand and agree that I am volunteering solely for personal purposes and without any promise or expectation of compensation, fees, benefits, or future employment with one·n·ten.  I further understand and agree that my volunteer service may be terminated at any time without cause or recourse. 

In consideration of being a one·n·ten Volunteer, I hereby release the sponsoring organizations, the host organization and facility, their boards, officers, employees, and volunteers (herein known as one·n·ten Staff) from any and all liability, claims, demands, or course of action whatsoever arising out of or relating to any loss, damage, or injury, including death, that may be sustained by me, or to any property belonging to me, whether caused by the negligence of the one·n·ten Staff or otherwise while participating in organizational activities, or while in, on, or upon the premises where the event(s) is being conducted. 


To the best of my knowledge, I am in good physical and mental condition, and I am not aware of any physical or mental infirmity that would place me at risk to participate in any way in one·n·ten activities. I voluntarily assume full responsibility for any risk of loss, property damage, or personal injury, including death, that may be sustained by me, or any loss or damage to property owned by me, as a result of being engaged in one·n·ten programs, whether caused by the negligence of the one·n·ten Staff or otherwise. I further hereby agree to indemnify and hold harmless the one·n·ten Staff from any loss, liability, damage or cost, including court costs and attorney's fees, that may accrue related to my participation in one·n·ten  programs, whether caused by the negligence of the one·n·ten Staff or otherwise. 


During the period of any participation of one·n·ten  programs, I hereby give permission for the one·n·ten staff to administer appropriate first aid and to authorize any medical attention or emergency treatment deemed necessary for me in the event of an accident, illness, or injury. I will be responsible for any and all costs of medical coverage and treatment provided not covered by personal insurance.  


It is my express intent that this Waiver of Liability and Hold Harmless Agreement shall bind the member of my family and spouse/partner, if I am alive, and my heirs, assigns, and personal representative if I am deceased, and shall be deemed as a release, waiver, discharge and covenant not to sue the one·n·ten Staff or agency. I hereby further agree that this Waiver of Liability and Hold Harmless Agreement shall be construed in accordance with the laws of the State of Arizona. In signing this release, I acknowledge and represent that I have carefully read this document and fully understand the contents, meaning, and impact of the agreement; sign this document freely and voluntarily without any inducement; I am at least 18 years of age and fully competent: and I execute this release for full, adequate, and complete.

Volunteer/Intern Non-Disclosure Agreement
Thank you for being a volunteer with one·n·ten. We are very grateful for your commitment and your contribution. As part of your important work with one·n·ten, you may be dealing with personal and sensitive information about our personnel, clients, and affiliates (i.e., addresses, personal or financial information, etc.) and one·n·ten as an organization. This agreement is designed to inform you about that protected information, and your signature indicates that you understand the importance of keeping this sensitive information within one·n·ten.

This agreement (the "Agreement") is entered into by one·n·ten and the volunteer listed below.

In consideration of the start of Volunteer/Intern's involvement with one·n·ten, the Volunteer/Intern and one·n·ten agree as follows:

1. Confidential Information

In the performance of Volunteer/Intern's job duties with one·n·ten, Volunteer/Intern will be exposed to one·n·ten's Confidential Information. “Confidential Information" means information or material that is valuable to one·n·ten and not generally known or readily ascertainable. This includes, but is not limited to:

a) Information about one·n·ten's participants, including addresses, emails, financial contributions, member lists, and member information;

b) Information concerning one·n·ten's financials, including cost information, profits, and accounting;

c) Information concerning one·n·ten's Staff and Volunteer/Interns, including salaries, strengths, weaknesses, and skills;

d) Any other information not generally known to the public, which, if misused or disclosed, could reasonably be expected to affect one·n·ten adversely.


2. Nondisclosure

Volunteer/Intern shall keep one·n·ten's Confidential Information, whether or not prepared or developed by VolunteerIntern in the strictest confidence. Volunteer/Intern will not disclose such information to anyone outside one·n·ten without one·n·ten's prior written consent. Nor will Volunteer/Intern make use of any Confidential Information for Volunteer/Intern's own purposes or the benefit of anyone other than one·n·ten. Volunteer/Intern's obligation to maintain the confidentiality and security of Confidential Information remains even after the Volunteer/Intern's employment with one·n·ten ends.

However, Volunteer/Intern shall have no obligation to treat as confidential any information which:

a) Volunteer/Intern is required to report as a Mandated Reporter:

b) Was in Volunteer/Intern's possession or known to Volunteer/Intern without an obligation to keep it confidential, before such information was disclosed to Volunteer/Intern by one·n·ten; or

c) Is or becomes public knowledge through a source other than Volunteer/Intern and through no fault of Volunteer/Intern.  
 
3. Return of Materials

When Volunteer/Intern's time with one·n·ten ends, for whatever reason, Volunteer/Intern will promptly deliver to one·n·ten all originals and copies of all documents, records, software programs, media, and other materials containing any Confidential Information. Volunteer/Intern will also return all equipment, files, software programs, and other personal property belonging to one·n·ten.

4. Signatures

Volunteer/Intern has carefully read all of this Agreement and agrees that all of the restrictions set forth are fair and reasonably required to protect one·n·ten 's interests and will abide by the standards outlined.
