'About You' Form (Volunteer Mentor)

On this form, we will ask you some questions about you before you come to your 1:1 meeting, so that we can get to know you better!

Please be honest with your answers. This will help us understand your background, experience and strengths so that we can give you the best possible support throughout your mentoring journey.

What motivates you to want to be a ReachOut Volunteer Mentor?

Have you had any experience working or volunteering with young people before? This can include volunteering, paid work experience or personal experience (i.e. with family members). If yes, can you tell us a little bit more about your experience?

Do you have any special skills or interests that you can bring into the sessions as a mentor? Some examples might include being fluent in a second language, being a sports coach, being an artist or performer etc.

Do you already hold a valid DBS certificate that you will be bringing to your 1:1 meeting? As a reminder, you can check if your DBS certificate is valid for the Volunteer Mentor role through here.

If you selected 'No' for the above question, please remember to fill in your DBS application by following the guidance on here before coming to your 1:1 meeting!

ReachOut may take and securely store appropriate photographs and/or video of mentors during ReachOut projects, and other project related activities such as workplace experience visits or graduation events. These photos or videos may be used in ReachOut publicity materials across our website, social media channels and print materials as well as shared with trusted third partners such as corporate partners or the media to promote ReachOut’s activities.

You may withdraw your consent at any time by emailing marketing@reachoutuk.org or speaking to your Volunteer Officer. In withdrawing consent, ReachOut will stop using relevant images in future materials immediately.

2. Prepare your 3 forms of ID (information on acceptable types of ID can be found here)

3. Attend your 1:1 meeting (details in the booking confirmation email / calendar invite)
