This form is to be filled out by BS/MPH staff and faculty advisors to submit a registration request on behalf of a student. STUDENTS MAY NOT SUBMIT THIS FORM. 

As a reminder, this form should ONLY be submitted to register the student for non-crossover courses. For all crossover courses, please direct students to register via GWeb.
