Thank you for completing the lesson registration request form. Space is limited and based on appropriate resources.

Physician’s statements & waivers must be completed within one year of the last form submitted.  You may print a form  from the website for the participant's doctor to complete, sign, date. Completed forms should be returned to the program director.

The LTR Program Director will contact you with a final schedule. Invoices for program service fees will be sent by email (check your SPAM folder) and due prior to the first day of the session.
