This form is setup for Arts Associations to acquire Association, Public Liability and Volunteer Insurance. Please complete the information below and select the required covers for that your association needs. Please take out all required covers now to ensure you have adequate cover for the year ahead.

Pricing for the covers is set depending on your associations gross annual income. Please select the options that is relevant for the size of your arts council needs to ensure you have the correct cover level in place.

Ensure you have your Association Details and Activity Excel completed and your No Claims Disclosure as these will be required at the end of this form.
