Submitting an Application:

  • By signing up to participate in the program December 12th, 13th, and 14th, the client understands that their personal data on this form is being shared with Denver Dream Center so it can effectively serve the needs of the child by connecting the sponsor with the resident Family.
  • We do our best to help as many families as we can but we can't guarantee that your family will be adopted by a giving family or receive gifts. Please check the box that you understand that by (1) completing this form and (2) if accepted, attending the Christmas in the City event, there is no guarantee that your children will be adopted or receive gifts for Christmas. 3) We only accept registrations for children from pregnancy to 17 years old.
  • I understand that I will not be accepted into the Christmas in the City program until I've been contacted (via phone, text, or email I provide) and notified by one of the DDC staff members.
  • I understand that when I am adopted that some of my information will be shared with the Adopting family, this includes my family name, my children(s) information I fill out in this form.
  • I understand that my children must be present the day of the event.
  • Each registration must have its own email and phone number. Please do not reuse your phone number or email if you are helping someone else register. If you do, your registration might be deleted. 
  • One submission per family. If you need to make changes after you submit you can follow instructions in the email you receive.
