Template Instructions (Hidden Section)
In Salesforce, create a new Campaign and populate it with all Current Member Contacts using the Member Contacts report (or any other invite group). Also, create Campaign Member Statuses for the values: Registered, Attended, Did not show, Withdrawn.

Copy the Campaign ID, and set as the default value for this field, Campaign ID, below: (if you can't see Campaigns in Salesforce, contact Salesforce Admin to enable Marketing User license)

  1. Click on the Campaign ID field below
  2. In the pop up menu, find the blue "Options" button
  3. The Field Properties menu on the left will appear - scroll down to "Default Value" and replace the Campaign ID placeholder with the ID from Campaign in Salesforce you just created.

Submissions will update Campaign Member records attached to this specific campaign and the Contact listed (Connector looks up Campaign & Contact). The Contact will Pre-Fill the first section so they can confirm. These fields are read-only.

If they choose to update records, these will not automatically update. You will be notified of their submission - you'll look out for those than answer "Yes"

The Contact is assigned within the Email Template set up to send the invitiation email. Work with the Salesforce team to ensure the Email Template link is set up! It should look something like:

You can customize 2 questions with the template for open text responses. Each is limited to 100 characters. Contact Salesforce team to make any changes. You can export these responses from Form Assembly using the Response Reports.

If you don't need both, simply hide the Questions instead of deleting them.
Make sure to update the Notifications section:
  1. Who should get response reports? Decide if PDF of response is necessary.
  2. What should respondents see when they submit? Acknowledgement or Redirect
  3. What email confirmation should get sent out after they submit?
