INFORMATION ABOUT THE SESSION

ACD is coordinating consultations with families and carers to inform a major review of Early Childhood Intervention (ECI) review and development of a best practice ECI framework.

Insight and ideas gathered from families and carers during these sessions will be consolidated into a report and framework for the ECI review.

All information will be de-identified and no names or identifying details will be published.

• I consent to the session being recorded for data analysis purposes
• I understand I need to attend for the full session to receive the voucher
• I consent to deidentified information provided in the session being used for the ECI Review report and framework or for advocacy
• I understand that my personal information and responses will not be shared with anyone else
