  • Please utilize this form to request the shipment of Community Engagement materials to your office/home location.
  • Orders are fulfilled on a Quarterly basis as follows:
    • January - March Submissions will be fulfilled in April
    • April - June Submissions will be fulfilled by July
    • July - September Submissions will be fulfilled by October
    • October - December Submissions will be fulfilled by January
  • The NYIC remains the right to decline the fulfillment of any requests. Completion of this request form does not guarantee your order will be fulfilled.
