The Bridge Application Form 2025

Program Description

New Heights has designed a new program to begin in March 2025. The Bridge Program (Pilot) is a sports-based youth development program that offers year-round in-house basketball programming. The Bridge will foster conditions for Brooklyn-based youth to thrive by supporting their academic preparedness, social and emotional growth, health and well-being, and gain visibility into sports careers.

Fee per participant: $100 payable upon acceptance into the program. Additional scholarships will be available to those for whom cost is a barrier to participating. You will NOT be charged any fees in this form. New Heights subsidizes the actual operating cost of $500 per participant.

Application Information
Completing this form begins your child's process of application to The Bridge Program (Pilot) season. Applicants will be notified of their acceptance or wait list status by Monday, March 10th, 2025. 

If your child is selected to participate in The Bridge, a parent or guardian will need to complete additional Parent/Guardian and Child registrations in the New Heights member portal in order to process the $100 payment and fully register the child. You will receive instructions on how to do this upon acceptance into The Bridge program.

The Application form should take about 15-20 minutes to complete. Please have your child’s insurance information available.

The demographic information requested on this form is collected in order to provide donors and institutions that support our programming with general information about the youth we serve. Your personal information will remain private and will never be shared. Please respond to each question honestly and completely

Consent and Commitment Requirements 

Parent/Guardian Consent

I, the undersigned, certify that I am the parent or legal guardian of my child. As such I grant New Heights Youth, Inc. permission regarding the following items: 


  • I do hereby grant permission for my child to participate fully in all New Heights programming and special events. 
  • I also grant permission for New Heights Youth, Inc and Bedford Courts I LLC to use the images (photographs, video, interviews, etc.) of my child taken by New Heights staff or volunteers, media representatives, newspapers, and television reporters, photographers, special event staff or public relations personnel. Photographs, videos and interviews will only be used to promote the New Heights Youth Program. 
  • I agree that my child may participate in online New Heights workouts, workshops and meetings with video (including Zoom, Microsoft Teams, Google Meet), and I agree that New Heights may record my child’s participation in such workouts, workshops and meetings, including her/his name, image and voice for program evaluation purposes, and to promote the New Heights program. 
  • I give permission for my child to complete all evaluative surveys to improve the New Heights program. I understand surveys may include questions regarding my child's mental, social and emotional well-being.  
  • I give consent for my child to participate in meetings with staff regarding their academic performance and general well-being. Parents will be informed if it is deemed beneficial to meet with their child on a regular basis to help with well-being or academic tutoring. 
  • I give permission for my child’s school to release academic records to New Heights Youth, Inc. 
  • I give my consent for my child to travel and otherwise participate in all trips with New Heights Youth, Inc. 
  • I give permission for my child to eat meals and snacks in connection with New Heights Youth, Inc programming. 
  • I give my consent to New Heights to seek medical care for my child should they need it. I agree to pay all of the costs (or use my insurance, if any) associated with the medical care that my child receives. I understand that New Heights will act in good faith to contact me before and after care is provided. 

  • I am fully aware that some program activities can be dangerous and may result in serious injury. Knowing and understanding all possible risks and dangers, including those resulting from the COVID-19 pandemic, I hereby expressly and willingly allow my daughter/son’s participation in New Heights’ program activities. In consideration of my daughter/son’s participation, I hereby waive all claims or causes of action against New Heights, its officers, directors, employees, agents, and volunteers from all liability in connection therewith. Furthermore, I give any New Heights parent, faculty, and/or staff and volunteers present at the program permission to seek medical attention for my daughter/son in the event that she/he becomes injured. 

New Heights Commitment Requirements

Commitment Guidelines
  1. All participants are expected to participate in all requisite New Heights programming: 

A.     Academic programming, High School and College Assist workshops, community service events, practices, games/tournaments, parent workshops, team meetings and appointments with staff (coaches meetings, tutoring appointment, mid-season check-ins, etc.). 

B.     Summer Academy and Week Away for all eligible middle school student-athletes (generally from July-early August). 

  1. All participants are expected to complete the enrollment and application process. This includes submitting all enrollment form components, including report cards and birth certificates.  
  1. All families and student-athletes must attend a family orientation meeting. 
  1. All participants are expected to submit report cards to New Heights within 7 days of receiving them. High school students should submit final transcripts by the end of July.  
  1. Please proactively communicate tardiness and absences to all programming (i.e., practice, game, academic session, workshop, fundraiser, etc.). 
  1. Student-athletes will be monitored if they receive more than one failing grade on a report card, or they receive below a 75% average. 
  1. Student-athletes and families are expected to initiate efforts to receive support by contacting New Heights staff, if student-athletes need academic support. 
  1. Arrive at least 10 - 15 minutes ahead of the listed start time for programming (practices, games/tournaments, fundraisers, etc.). 
  1. Respect all coaches, staff, teammates, and property. 
  1. New Heights student-athletes are not permitted to participate in any programming that conflicts with mandated summer school or other mandated academic events at their respective schools. 
  1. If New Heights hosts online programming, student-athletes and families are expected to join with their video cameras on. 

  1. All New Heights student-athletes are expected to participate in their respective school’s athletic program, unless there are circumstances limiting their involvement, which have been rehearsed with their coach and/or athletic director. 



Boys Club New York Waiver 

The Major R. Owens Community Center is a multi-sport facility. Under some circumstances, a minor under 18 years old may be approved to use the fitness center as part of New Heights Youth, Inc. basketball program. Minors will be accompanied and supervised at all times by a New Heights Youth, coach, or instructor 18 years or older. In consideration of facility use (or the use by my minor child or other minor for which I am a guardian) of the exercise equipment and facilities provided by BedfordCourts I LLC, Bedford Courts Local Development Corporation, and Boys Club of New York, Inc., I expressly agree and contract, on behalf of myself (or, if applicable, my minor child or other minor for which I am a guardian), and assigns, that Bedford Courts I LLC, Bedford Courts Local Development Corporation, Boys Club of New York, Inc., New Heights Youth, Inc., and each of their insurers, employees, officers, directors, members, principals, associates, parents, subsidiaries, and affiliates (collectively, the “Company”), shall not be liable for any damages arising from personal injuries (including death) sustained by me (or, if applicable, my minor child or other minor for which I am a guardian), or my guest in, on, or about the premises, or as a result of the use of the equipment or facilities, regardless of whether such injuries result, in whole or in part, from the negligence of the Company. Minors must be approved by the major R. Owens CommunityCenter management staff and the ExecutiveDirector/AssociateExecutive Director of New Heights Youth, Inc. to enter the weight and cardio area.
 
This form must be completed and signed to verify parental/guardian consent. Access to the fitness center is a privilege and appropriate conduct is expected. If a minor is observed to be attending without supervision, violating fitness center rules, or acting inappropriately, the New Heights Executive Director will be notified and access to the fitness center may be suspended. This policy is subject to change at the discretion of the Major R. Owens Community Center management staff.
 
WORKOUT ATTIRE
Appropriate workout attire is required in the gym and cardio area. shirts, shorts or sweat pants, and athletic shoes must be worn at all times in the fitness center. It is also expected that all fitness center participants observe proper hygiene and limit the use of perfumed sprays.
 
CONSENT I, the parent/legal guardian, have been provided with the ParentConsent Cardio and Weight AreaForm for New Heights Youth, Inc., and have read, understood, and signed for my minor child/person. I understand that the adult designated to attend with my minor child/person must also be a coach/instructor of New Heights Youth. I understand the nature of the Cardio and Weight area privilege, and that my child is responsible for his/her behavior at all times. I understand that any violation of Weight and Cardio area privileges rules may result in loss of privileges.
 
 
By the execution of this agreement, I accept and assume full responsibility for any and all injuries, damages (both economic and non-economic), and losses of any type, which may occur to me (or, if applicable, my minor child or other minor for which I am a guardian) or my guest, and I hereby fully and forever release and discharge Bedford Courts I LLC, Bedford Courts Local Development Corporation, Boys Club of New York, Inc., New Heights Youth, Inc., and its insurers, employees, officers, directors, and associates, from any and all claims, demands, damages, rights of action, or causes of action, present or future, whether the same be known or unknown, anticipated, or unanticipated, resulting from or arising out the use of said equipment and facilities.
 
I expressly agree to indemnify and hold Bedford Courts I LLC, Bedford Courts Local Development Corporation, New Heights Youth, Inc., Boys Club of New York, Inc., the City of New York and its agencies and each of their respective owners, parents, subsidiaries, affiliates, members, principals, officers, employees, and agents harmless against any and all claims, demands, damages, rights of action, or causes of action, of any person or entity, that may arise from injuries or damages sustained by me (or, if applicable, my minor child or other minor for which I am a guardian) or my guest. By signing (or, if applicable, my minor child or other minor for which I am a guardian) I agree to comply with all rules imposed by the company regarding the use of the facilities and equipment.
 
By signing (or, if applicable, my minor child or other minor for which I am a guardian) agree to conduct themselves in a controlled and reasonable manner at all times and to refrain from using any equipment in a manner inconsistent with its intended design and purpose. I understand and acknowledge that the use of exercise equipment involves risk of serious injury, including permanent disability and death. I understand and agree that Bedford Courts I LLC, Bedford Courts Local Development Corporation, Boys Club of New York, Inc., and New Heights Youth, Inc., are not responsible for property that is lost, stolen, or damaged while in, on, or about the premises.
