The primary purpose of this document is to establish an understanding between the selected Veteran, the Veteran's family and Purple Heart Homes, Inc. (PHH) relative to media exposure that will be generated as a result of outreach to the community where the Veteran's home shall be modified or constructed. Purple Heart Homes is a non-profit 501 (c)3 organization, which is dependent on voluntary contributions from individuals, sponsors, corporations, and foundations that enable PHH to build each specially adapted barrier free homes for not only the Veteran signing this agreement but additional seriously injured Veterans as well. Therefore the use of the Veteran's Image, likeness or story for the purposes of promoting, fundraising, advertising opportunities performed by Purple Heart Homes (PHH) any and all media channel outlets including newspapers, magazines, local TV, national TV, social media outlets such at YouTube, Twitter, Facebook, Flickr, etc. is granted upon my signature to this agreement.
Media Releases Issued Using the Veteran's Name:
Purple Heart Homes issues multiple media or more media advisories:
- To encourage all members of the media in the Veteran's local area to attend an informational event to learn about Purple Heart Homes and the Veteran's specially adapted home that will be built over a six month period.
- A groundbreaking release with the Veteran's name included in the media alert.
- The next media advisory invites skilled trades' people to participate in a construction blitz that includes the Veteran's name to seek participation and involvement from the community to help make the modifications.
- Members of the press are extended an invitation to cover the completion of the Veteran's home in a formal Ceremony.
- Two primary names are included in all media releases - Purple Heart Homes and the Veteran's name with a description of the severe injury incurred while serving the in United States military.
- Members of the media will want to interview the Veteran and family members to support PHH story being told to encourage and involve the community, skilled trades' people, volunteers and corporate sponsors to help build and complete the Veteran's home.
The Veteran and their family agree that any media inquiries that come directly to the Veteran and or their family shall be forwarded immediately to PHH. All media shall be coordinated by PHH. PHH respects the privacy of the Veteran and the Veteran's family and will not provide the media with a contact phone number or email address without first obtaining permission from the Veteran and family members.
- ➤➤ I grant permission to PHH, to use my name and/or photographs for use in PHH publications such as recruiting brochures, websites, newsletters, and magazines, and to use my name/and or photographs on display boards, and to use my name and/or photographs in electronic versions of the same publications or on the PHH web site or other electronic forms or media.
- ➤➤ I hereby waive any right to inspect or approve the finished photographs or printed or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the photograph.
- ➤➤ I hereby agree to release, defend, and hold harmless PHH and sponsors, volunteers, trades people, including any firm publishing and/or distributing the finished product in whole or in part, whether on paper or via electronic media, from and against any claims, damages or liability arising from or related to the use of the photographs, including but not limited to any misuse, distortion, blurring, alteration, optical illusion or use in composite form, either intentionally or otherwise, that may occur or be produced in taking, processing, reduction or production of the finished product, its publication or distribution.
